SCHOLARSHIPS OFFERED THROUGH THE OFFICE OF DISABILITY SERVICES
Purpose:
The Office of Disability Services will be awarding scholarships from The Carol
A. Douglas Endowment for Disabled Students and The Lion’s Foundation of the
Piedmont Fund. The purpose of each scholarship is to provide financial
assistance to four undergraduate students and one graduate student attending UNC
Charlotte who have a documented disability and are registered with the Office of
Disability Services. In addition to these basic requirements, students must meet
the eligibility requirements listed below.
Eligibility Requirements:
1. Cumulative GPA at UNC-Charlotte above 2.5
2. Documented Financial Need
a. Completion of FAFSA or equivalent
3. Verification of graduate or undergraduate enrollment at UNC Charlotte with
registration and maintenance of a minimum of 6 credit hours for undergraduate
students and 3 credit hours for graduate students
4. No disciplinary actions on academic record
5. Submission of completed application packet
6. Signed release of attached Consent to Release Information

Scholarship Award:
The scholarship application acceptance period will be January 9-March 10, 2008.
Winners will be notified by mid-April 2008. The scholarship money will be
awarded to the selected recipients the 2008/2009 academic year. This scholarship
is not renewable, but students may reapply for the scholarship each Spring
semester.
Selection of Scholarship Award Recipient(s):
The person(s) selected for the scholarship awards will be chosen by the
scholarship selection committee, which is comprised of several UNC Charlotte
faculty and staff who serve students on the UNC Charlotte campus in a variety of
settings.
Award Announcements:
Scholarship award winners will be notified by letter. Award winners may also be
announced on the Office of Disability services website, Office of Disability
services news board, The University Times, Tapestry newsletter, the faculty
newsletter, The Charlotte Observer, and other local media outlets. The Piedmont
Chapter of the Lion’s Club and Carol A. Douglas will also be notified of
scholarship award recipient(s).
PLEASE ATTACH THE FOLLOWING ITEMS TO COMPLETE THE APPLICATION
PROCESS:
1. PERSONAL STATEMENT/ESSAY
Please attach a personal statement/essay of 500 words or less to be given to the
scholarship selection to committee to help them gain a better understanding of
your background. Please include the following information:
• How your disability has impacted your life and its effect on you in the
academic setting
• Why you would benefit financially by receiving this scholarship. Please
address any specific financial needs that may not be met otherwise
• Any additional information regarding your ambitions, goals, leadership roles
and any other factors you would like to be considered during the selection
process

3. RÉSUMÉ

4. TWO PROFESSIONAL REFERENCES
Please see attached guidelines on pages 5-6 to give to the persons providing
your reference letters. Reference letters must be included in the application
packet in sealed envelopes with reference persons’ signatures over the sealed
envelopes.
5. FINANCIAL NEED DOCUMENTATION
a. FINANCIAL AID REWARD LETTER
This document can be obtained through BANNER.
b. STUDENT AID REPORT (SAR)
This is the document that the student receives after the FAFSA form is
processed.
c. FEDERAL APPLICATION FOR FREE STUDENT AID (FAFSA)
The FAFSA is the first step in the financial aid process and is used to apply
for all federal student aid money. Learn more at: www.fafsa.ed.gov
