Testing Accommodations
All accommodations are determined based upon the individual and their
documented disability.
TESTING IN THE DS OFFICE
Eligible students can receive testing accommodations
within the Office of Disability Services. We have
several small testing rooms that are utilized for a lower distraction environment
and for access to assistive technology. Space is
limited so the testing procedures must be followed!
If testing accommodations within DS are appropriate,
a Test Administration Form must
be completed and returned to the Office of Disability Services.
TEST ADMINISTRATION FORM
Students are provided with the
Test Administration form to
give to their professor and once completed,
the student ensures that the form is returned to the Office of Disability
Services. This form provides all the information regarding the student’s exams.
Request forms for testing in the DS office should be received at least one week
prior to the exam. The form is in triplicate. The
professor keeps the yellow copy and the student keeps the pink copy for their
records. The white, top copy must be given to the DS office.
Faculty indicates all exam dates for the
semester on this one form. Please make sure all information, including the
allotted exam time, items/materials permitted during
testing, and other comments have been provided.
It is the student's responsibility
to ensure the Office of Disability Services has the Test Administration form
prior to exams. Students must
contact us to verify the form has been received in advance of the test.
CHANGES WITH TESTING DATES
Notify our office immediately regarding any
changes to exam schedules.
FINALS
Requests for taking finals in the DS office require several
weeks notice. Deadlines will be made available by the DS staff.
PROCTORING IN DS
All tests given in the Office of Disability Services are proctored and
monitored with surveillance cameras.
TESTING IN THE CLASSROOM/WITH FACULTY
Discussion with faculty and staff
is
critical to receiving testing accommodations within the
classroom or with the faculty/staff member.
