Students' Responsibilities
Students registered with the Disability Services Office are active
participants in the accommodation process. Responsibility and self advocacy are
integral to acquiring and maintaining accommodations. The following information
is provided to assist students in securing their accommodations:
*Students should notify the Office of Disability Services prior to the
scheduled time if they can not attend an appointment.
*Students must provide the Letter of Accommodation to their faculty in order
to initiate accommodations.
*Students benefit from discussing their accommodations with their professors
at the time of delivering the Letter of Accommodation.
*If accommodations include books in alternate formats, interpreters,
classroom furniture, and classroom location, contact the Disability Services
office immediately after registering for classes and prior to the start of the
semester.
*A week’s notice is required in order to test in the DS office. If our office
does not have the request form for test administration, we can not administer
exams. It is the student’s responsibility to bring the white Request for Test
Administration form copy back to the DS office after discussing testing
accommodations with their professors. Students should keep the pink copy for
their own records.
*When testing in the DS office all personal belongings must be placed in
available lockers or in the front office. This includes jackets, cell phones,
drink bottles, book bags, PDA’s etc. If a calculator is allowed for an exam, the
Disability Services Office provides one unless otherwise notified by the
student’s instructor.
*Disability Services contacts faculty for instructions if students are over
15 minutes late for an exam.
*If approved for note taking services, students must turn in the request form
to their professor. Students are asked to pay attention for the Note Taker
Request announcement to be made by the instructor and to check that a note taker
has volunteered. If notes are not being placed in the assigned folder after one
week, students must inform the office of Disability Services.
*All books on tape from RFB&D and borrowed equipment must be turned in at the
end of each semester.
*If a student experiences absences as a result of their disability, they
should notify their instructors and the Office of Disability Services
immediately. Additional documentation will be required.
*Students who acquire an additional disability while
already registered with the Office of Disability Services will be asked for documentation in order to
provide accommodations for the new diagnosis. Please visit the
