Note Taker Program
Volunteer note takers are provided for students with disabilities in classes and
labs when appropriate. Like all disability related services, the note taker
program is confidential, using a numeric system to maintain privacy for students
with disabilities. Students who wish to self-disclose may choose to introduce
themselves to the note taker volunteer. Otherwise, identities should remain
confidential.
The note taker service is used as a supplement to the notes that the student
with a disability can take in class.
Student Responsibilities:
Complete check-in early for semester accommodations
Provide a letter of accommodation and a note taker request form to the
instructor
Attend all classes unless there is a medical/emergency excuse
Take notes in class
Pick up notes weekly
Do not share supplemental notes with other students
Notify DS office if notes are not received after two weeks
Notify DS office if withdrawing from the class or if notes are no longer needed
Faculty Responsibilities:
Do not request a note taker for a student without a note taker request form
If there are no student volunteers after the first request, make a second
request to the class
When making an announcement for a note taker, do not reveal the eligible
student’s identity
Due to the legal guidelines for note taking services, please announce the need
for additional note takers when indicated by multiple request forms
Faculty are NOT required to provide their own notes to a student
